Reaching Home Funded Projects

The following service projects received Reaching Home funding for the period April 1, 2026 to March 31, 2028:

• Access Youth Outreach Services Society – YEW/FS Housing Prevention & Stabilization Stream
  This project serves youth in the Tri-Cities, particularly Coquitlam, experiencing or are at imminent risk of homelessness by providing housing placement, housing loss prevention, and shelter diversion services.
• Aunt Leah’s Independent Lifeskills Society – The Link: Housing First – Continuum of Support for Youth Aging Out of Government Care
  This project serves youth from the foster care system experiencing or at imminent risk of homelessness in the Metro Vancouver region by supporting those entering difficult rental markets and developing essential life skills.
  
• Canadian Mental Health Association of North & West Vancouver – Outreach Services and Homelessness Prevention
  This project will serve individuals 19+ with mental or physical health challenges experiencing or are at imminent risk of homelessness by providing housing placement, housing loss prevention, and shelter diversion services.
• CityReach Care Society – CityReach Case Management
  This project serves individuals in the Tri-Cities, Surrey, and Delta experiencing or at imminent risk of homelessness through housing loss prevention and shelter diversion services.
• Coast Foundation Society – Breaking the Cycle: Housing First After Custody
  This project serves individuals with complex needs, exiting correctional facilities across Metro Vancouver who are experiencing or are at imminent risk of homelessness by providing housing placement, prevention (pre-release planning) services as well as and client supports.

• Collingwood Neighbourhood House Society – Staying Housed in Collingwood
  This project serves individuals and families in Vancouver and Burnaby experiencing homelessness or at risk of homelessness.
• Community Builders – Community Builders Outreach Program
  This project provides safe and supportive low-barrier housing and shelter to individuals experiencing homelessness or barriers to accessing housing in Metro Vancouver and Richmond.
• Community Impact Real Estate Society  DTES Peer-Led Support Services Program
  Peer workers will conduct outreach to around Vancouver and the Downtown Eastside with target populations of Indigenous individuals, women and those engaged in sex work, those living in SROs, and those living on the street and encampments.
• Connective Support Society – Community Integration Through Housing in the Greater Vancouver Area
  This project will serve individuals who have a history of homelessness and a history of criminal legal system involvement in the Metro Vancouver region by providing housing placement and prevention services as well as Indigenous cultural supports as needed.

• Covenant House – Vancouver’s Youth Transitions Project
  This project will reduce homelessness amongst youth ages 16 to 24 and transition them to adulthood by providing life skills that lead them to independence.
• Daughters of Hope Resources Society – Reaching Home Housing Connections
  This project will address the unique challenges experienced by Black, racialized, newcomer, and refugee individuals and families accessing housing who are or are at imminent risk of homelessness in the Surrey and Langley area.
• DIVERSEcity Community Resources Society – Safe Harbour: Transitional Housing & Stabilization for Youth Fleeing Violence
  This project aims to provide stabilization-based housing and trauma-informed supports for youth fleeing violence in Surrey and the lower mainland.
• Downtown Eastside SRO Collaborative Society – The SRO Hub
  The SRO Hub empowers tenants of privately owned single room occupancy buildings (SROs) in the Downtown East side to improve habitability and prevent evictions related to conditions in tenant’s rooms.
• Downtown Eastside Women’s Centre Association – Downtown Eastside Housing Services for Vulnerable Women
  This project will serve women and their children, including Indigenous and racialized women in the Downtown Eastside, who are or are at imminent risk of homelessness though housing placement, prevention, and shelter diversion services.
• Encompass Support Services Society – Youth Housing Placement Outreach and Support
  This project will serve youth and youth aging out of care who are or are at imminent risk of homelessness in Surrey and Langley.
• Family Services of Greater Vancouver – Directions Emergency Housing Fund
  Directions Housing Subsidy & Support Fund is a low-barrier program for youth in Metro Vancouver experiencing or are at imminent risk of homelessness.

• Hogan’s Alley Society – Support for Vulnerable Metro Vancouver Black Populations 2026-2028
  This project will support Metro Vancouver’s Black and African descent community who are or are at imminent risk of homelessness.
• Hollyburn Community Services – Seniors, Families, and Working Age Individuals 31 and Older
  This project will serve seniors 55+, families, and working-age adults across the North Shore and surrounding areas who are or are at imminent risk of homelessness.
• Hollyburn Community Services – Reaching Home Youth Services
  This project will serve youth ages 13 to 30, including Indigenous youth and those exiting institutions in the North Shore and Metro Vancouver region who are or are at imminent risk of homelessness.
• Hopelessness Ends Lives Progress Society – HOPES Program (Housing, Outreach, Prevention, & Empowerment for Seniors)
  This project serves seniors 55+ in Metro Vancouver experiencing or are at imminent risk of homelessness and help maintain housing stability and improve social and community integration.
• InnerVisions Recovery Society of BC – InnerVisions: Expanding Men’s Transitional Housing & Enhanced Wrap-Around Services in Maple Ridge
  This project provides transitional housing and enhanced wrap-around services in Maple Ridge for men who are or are at imminent risk of homelessness and have complex needs, including substance use, and mental health challenges.
• Journey Home Community Association – Homeless to Housed: Welcoming Refugees with Housing and Dignity
  In partnership with Kinbrace, this project will serve homeless refugee claimants with transitional housing as well as moving to, and securing permanent housing, in addition to other integration services.

• Kettle Friendship Society – The Kettle’s Homeless Prevention Project
  This project serves individuals experiencing or are at imminent risk of homelessness and promote housing stability as well as economic, social, and community integration.

• La Boussole Centre Communautaire Societe – Francophone’s social services to reduce homelessness overall in the Lower-Mainland
  La Boussole aims to reduce chronic and overall homelessness by providing housing placement, housing loss prevention and shelter diversion, as well as support services to 150+ Francophones in BC.

• Lower Mainland Purpose Society for Youth and Families – Housing Support Program
  This project serves individuals and families who are or are at imminent risk of homelessness with housing placement and housing loss prevention services in the New Westminster area.

• Muslim Food Bank and Community Services Society – Building Pathways to Stable Housing for Racialized and Equity-Deserving Groups
  This project aims to reduce homelessness in Metro Vancouver, Surrey, and Langley among racialized and equity seeking groups with a two-tiered approach, direct support and follow-ups.

• Pacific Community Resources Society (Surrey) – Surrey Youth Reaching Home
  This project serves youth in Surrey who are or are at imminent risk of homelessness by providing housing placements and housing loss prevention services. This project will also help youth secure and maintain housing by providing access to education, life skills, healthcare, employment, cultural programing, and social connections.

• Progressive Housing Society – Burnaby Housing and Outreach Hub
  Progressive Housing Society serves individuals in Burnaby and New Westminster who are chronically homeless and at imminent risk of homelessness by offering comprehensive, no-barrier housing and other resources.

• RainCity Housing and Support Society – Surrey Housing First ICM Team
  This project serves people who have experienced chronic homelessness primarily in Surrey and offers outreach to chronically, episodically homeless individuals to acquire and maintain suitable housing and create goal-oriented service plans.

• Senior Services Society of BC – Seniors Housing Services
  This project serves seniors 60+ who are or are at imminent risk of homelessness by providing housing placement and housing loss prevention services in the Metro Vancouver region.

• SHARE Family and Community Services – Tenant Advocacy Program
  The program will support individuals experiencing or at imminent risk of homelessness in Coquitlam, Port Coquitlam, and Port Moody through housing placements, housing loss prevention, and shelter diversion services.

• Sources Community Resources Society – Essentials to Housing
  Essentials to Housing will provide those living in Surrey, Delta, and Langley experiencing or are at imminent risk of homelessness through housing placement, housing loss prevention, and shelter diversion services.

• Sources Community Resources Society – Essentials to Housing: Peninsula
  Essentials to Housing: Peninsula will serve individuals and families in South Surrey and White Rock who are or are at imminent risk of homelessness by providing housing placement, housing loss prevention, and shelter diversion services.

• Stepping Stone Community Services Society – Stepping Stone: Reaching Home
  This project serves the population of Surrey and Langley with a focus on seniors 55+ experiencing or are at imminent risk of homelessness though housing placements, housing loss prevention, and shelter diversion services.

• Surrey Women’s Centre Society – Violence Recovery & Housing Resilience Program
  This project will serve women in Langley who are or are at imminent risk of homelessness by providing housing placement, housing loss prevention, and shelter diversion services. Additionally, it will provide low-barrier, wraparound support through drop-in and mobile services for survivors experiencing gender-based violence in order to increase access to safe housing.

• The Realistic Success Society – Recovery Works: From Homeless to Housed
  This project serves homeless men exiting the criminal justice system living with the addictions. The aim is to reduce homelessness in Surrey by connecting individuals prior to their release with transitional peer support.

• The Elizabeth Fry Society of Greater Vancouver – The Delta Collaborative: Home Bound
  The Delta Collaborative takes a recovery-oriented approach to quickly move people, from homelessness into independent and permanent housing, in the Surrey and Delta region. This often includes discharge planning from Surrey pre-trial, prisons, recovery houses, and treatment facilities.
• The Last Door Recovery Society – Last Door Transitional Housing Support Program
  This project serves men exiting addiction treatment services in Metro Vancouver who are at imminent risk of homelessness.

• Turning Point Recovery Society – Community Response to Homelessness in Richmond BC
  This project will serve adults in Richmond who are experiencing or are at imminent risk of homelessness, including individuals with multiple barriers such as low income, health concerns, tenancy challenges, and limited support networks.

• Unlocking the Gates Services Society – UTG Warrant Resolution Program
  This project seeks to support homeless individuals with active warrants by working with BC Probations and the Ministry of Social Development and Poverty Reduction. The project will provide access to legal support, housing services, and basic care to those in Metro Vancouver.

• Vancity Community Foundation – Granting to Bridge the Gap
  This subproject will provide 1320 non-repayable grants for rent or essential utilities to prevent individuals at imminent risk of homelessness in the Greater Vancouver region from becoming homeless.

• Westminster House Society – Primary Care Program
  This project serves women in New Westminster who are experiencing or are at imminent risk of homelessness through housing placements, housing shelter loss prevention, and shelter diversion.

• WISH Drop-In Centre Society – Low-Barrier Housing Support for Chronically Homeless Women and Gender Diverse Street-Based Sex Workers in Vancouver’s Downtown Eastside
  WISH serves individuals experiencing or are at imminent risk of homelessness by providing housing placement, housing loss prevention, and shelter diversion services as well as ongoing housing stabilization and client supports such as access to education and job training programs.

• YWCA British Columbia – YWCA Crabtree Corner: Supportive Transitional Housing in the Downtown Eastside
  This project will serve women and children in Metro Vancouver experiencing or are at imminent risk of homelessness and/or escaped domestic violence by providing housing placement, housing loss prevention, and shelter diversion services.
